What is INR used for?
Determining how long it takes for blood to clot using prothrombin time for people who take warfarin.
Who is warfarin prescribed to ?
People with AF, risk factors for having a stroke, DVT, PE, artificial heart valves
What is the target INR?
A value which is high enough to prevent clots, but not so high that it’ll cause bleeding
What does it mean if a patient’s INR is above target?
There blood is clotting too slowly and there’s a risk of bleeding
What does it mean if a patients INR is below target?
They’re at risk of clotting
Which foods affect warfarin?
PY450 inducers, rich in vitamin K.
Leafy greens
chickpeas
liver
egg yolks
mature cheese
avocado
olive oil
What dietary advice should be given to people who take warfarin?
Keep your diet consistent.
What foods should a person on warfarin avoid?
cranberry juice and grapefruit
Which medications have an interaction with warfarin?
Amiodarone
NSAIDs
Cholesterol-lowering
antibiotics such as erythromycin
miconazole, fungal infections such as thrush
Which symptoms should you ask about in INR Counselling?
Bruising, bleeding
Which medications should you ask about in INR counselling?
