How is amount alcohol calculated
Units no more 14 a week
Depends on size and strength of drink
Single measure spirits 1.4 units
Glass wine 1.4 units, 8.3 units bottle wine
40 units litre vodka
1 unit 8g pure alcohol
Volume x ABV%/1000=units
Hazardous drinking
Drinking above level that may cause harm in future but is not currently causing clear evidence of harm
Harmful drinking
Drinking at a level leading to current evidence of physcial, social or psychological harm
Alcohol dependence
Defined on ICD10as individual having 3 or more of a range of symptoms of alcohol dependence- tolerance, withdrawal, craving, difficulties in controlling drinking , neglect of alternative pleasures, persistent drinking despite negative consequences
Medical and social harms with hazardous and harmful drinking
Medical: injuries, mental and behavioural disorders, GIT conditions, cancers, CVD
Social: not sleeping, marital harm, child abuse, crime/voilence, homicide
Steps involved in brief intervention for hazardous and harmful drinking
Reference to screenings test results concerning frequency, amount, pattern of drinking and problems experienced in relation to drinking
Copy leaflet shown to patient
Review each panel in sequence
Give feedback, provide info, establish goal, give advice limits, review what’s a standard drink, conclude with encouragement
Barriers to implementing brief intervention
Lack of time
Inadequate training
Fear of antagonising patients
Perceived incompatibility of alcohol counselling with primary healthcare
Belief that ‘alcoholics’ don’t respond to interventions
Alcohol use disorder identification test AUDIT
Questionnaire to detect at risk, harmful or heavy drinking
10 questions covering three domains of hazardous alcohol use, alcohol dependence’s harmful alcohol use

Users normally do better when: person conducting interview is friendly not threatening, aim of the questions clearly related to diagnosing users state of health, user doesnt consume alcohol or drugs while selection is made, info considered confidential, questions easy to understand
