Plaque control record (O’Leary - PCR) formula
sum of disclosed surfaces divided by total surfaces (available) x 100% = % index (plaque score)
record in red surfaces with disclosed plaque
Patient hygiene performance index (PHP)
- each tooth receives score out of 5
PCR scoring
total # of teeth x 4 (or 6)
count # of surfaces with biofilm x 100, divide total available surfaces by amount of surfaces covered
- 10% or less good
Green stain
chromogenic bacteria and fungi, extrinsic, should not be scaled (b/c demineralized enamel)
Black stain
iron in saliva, extrinsic, scale and polish
Black-line stain
iron, in healthy women w good oral hygiene, extrinsic, scale and polish
orange stain
chromogenic bacteria from poor oral hygiene (rare), extrinsic, scale and polish
brown stain
tobacco, food, beverage or chemo, extrinsic, scale and polish
grey/brown - green stain
marijuana, extrinsic, scale and polish
yellow stain
Oral biofilm, poor oral hygiene, extrinsic, client remove with brush
blue - green stain
mercury and lead dust, extrinsic, scale and polish
Red-black stain
betel nut/leaf, extrinsic, scale and polish
white-spotted to brown-pitted enamel stain
dental fluorosis, excessive Fl during enamel development, intrinsic, cannot be removed
White spots on enamel stain
hypo calcification, high fever during enamel formation, intrinsic, cannot be removed
white brown spots on enamel may be smooth or rough stain
acid erosion of enamel caused by biofilm, intrinsic, cannot be removed
