What are some common safety guidelines for pt’s elimination?
- When possible, place pt’s on toileting schedule
- When digital removal of impacted fecal material is ordered you MUST take baseline vitals and monitor heart rate through the procedure
- Always use standard protocol when handling fecal matter
What are the 2 types of bedpans?
1) Regular bed pan (big one) = usually for independent pt’s
2) Fracture bedpan (smaller one) = immobile pt’s
What are the types of feces? From the Bristol stool form
1) Difficult to pass, separated hard
2) Sausage shaped but lumpy
3) Sausage with cracks
4) Smooth and soft
5) Soft blobs (passed easily)
6) Fluffy pieces
7) Watery, no solid (entirely liquid)
Define fecal impaction
Inability to pass a collection of hard stool. Occurs in all age groups
When are digital removal performed?
When enemas and suppositories are not successful. This procedure requires an order

Help patient to a left side-lying position with knees flexed and back.
When do you stop with the enema?
Enemas until clear

1) Budded stoma = colostomy
2) Retracted stoma = ileostomy
