What do we compare in right to medication?
Compare the prescribers orders to the MAR when medication is initially ordered
What do orders need?
Pt name, DOB, date prescribed, medication name, strength and dosage, route, dose frequency, and time of administration
What is one time medication?
Comes in 2 forms either stat or now
Stat = used in emergencies which is given IMMEDIATELY and only once
Now =need quickly but not stat usually 90 minutes to give but depends on employers’ policy
What information is NOT used to confirm pt identity?
room number
What do orders via verbal or telephone require and who takes these orders?
- A “repeat back” of the complete order or test
- Only authorized employees can receive and record prescriptions that are not written
- MUST document time and date and order as well as HCP name if via verbal order or telephone
How do we determine the right dose and how many times do we check medication?
- TRIPLE checking
- Packaging has dose. Check MAR and order to confirm you have the right one
What form of medication errors are really common?
medication routes is where most mistakes are made
What happens if medication is missing the route?
Consult prescriber immediately
What is the window for giving medication?
Time sensitive usually within 30 minutes and non-sensitive 1-2 hour window BUT it all depends on the employer.
What are 2 identifier for the correct patient?
Name AND DOB
How do you document for medication?
Document once you have given the medication. Include the name of med, route, time, dosage on patient MAR
Why right of assessment, evaluation and education?
- Assessment: is the med safe and appropriate to give?
- Education: asking about allergies
- Evaluation: assess for side effects, effectiveness
What are the 3 principles by the CNO outlined related to medication practices promote public protection

What units do we use for medications?
