Describe the function of tacrolimus
Tacrolimus inhibits calcineurin and inhibits production of cytokines
Describe the function of sirolimus and everolimus
They inhibit mTOR and inhibit T cell response to cytokines
What are the side effects of tacrolimus?
Nephrotoxicity, neurotoxicity, hypertension
What are the side effects of sirolimus and everolimus?
Hepatotoxicity (liver), myelosuppression (thrombocytopenia)
Describe the function of leflunomide
Inhibits pyrimidine synthesis; very teratogenic
Describe the function of mycophenolate mofetil
Inhibits IMPDH (de novo purine synthesis)
What are the side effects of TNF-alpha inhibitors?
Reactivation TB
What is the preferred combination of medication for maintenance immunosuppression?
antiproliferative agent, corticosteroid, calcineurin inhibitor
