Where is an incident report completed?

A

An incident report is completed whenever there is an unusual occurrence

  • Service delays- that may negatively impact the pt
  • Delay accessing pt
  • Prolonged scene time
  • Suspicious or unexpected death that are likely to be investigated by the coroner/ police
  • Any circumstances that resulted in harm to a pt or any other person in the ambulance
  • Equipment deficiencies
  • Any health and safety risks to pt or others being transported
  • When the service is going to receive a complaint

Who gets a copy of the incident report?

A
  • The service and Emergency Health Service Branch of the MOH

What will the incident report include?

A
  • Call number
  • Description of the actions and events requiring the completion of the incident report
  • A description of each EMA’s or paramedic’s observations, actions and procedures not documented on the ACR in relation to the event
  • A detailed description of the scene
  • A copy of the ACR

What to do when equipment malfunctions?

A
  • Identify of the make and type of equipment, including any identifying number (ex. model and serial number);
  • A description of the nature and timing of the equipment failure or malfunction;
  • Description of the impact, if any, of the equipment failure on pt care
