Why do we need to temporise teeth?
As Endodontic procedures are time consuming and often cannot be completed in one visit
So we need to maintain a clean environment in the canals between appointmtens
Why is it important we temporise between appointments?
As we have just disinfected the canals so want to maintain this clean environment and avoid bacteria in the mouth re entering the canal
What are the 2 parts of temporisation
- Inter appointment dressing
2. Temporary restoration
Why do we need an inter appointment dressing
Having extirpated the pulp, we need to place a medicament in the canal space to maintain the disinfected environment
Giver examples of some inter appointment dressing
Non setting calcium hydroxide
What are the benefits of using non setting calcium hydroxide
- It has a high pH so is antibacterial and stimulates healing of boney lesions
- Is radio opaque
How do we place non setting calcium hydroxide into a tooth?
It is pre mixed in a syringe
We add a little bit onto the orifice of the canal and then try too spread it into the canal with paper points or files
Why do we not just inject non setting calcium hydroxide straight into the canal?
As we risk it extruding beyond the apex
After placing non setting calcium hydroxide into the canal what do you do?
Insert and place a cotton wool pledget into the canal
Why do we need to place a cotton wool pledget into the canal?
It prevents temporary restorative material from lodging in canal orifice
What can happen if you forget to place a cotton wool pledget after nonsettign calcium hydroxide into a canal?
You risk the restorative material lodging into the canal orifice
This can make it harder to tremolo the temporary restorative material without damaging the tooth
What do we sometimes soak the cotton wool pledget in before placing it into the canal?
Ca(OH)2 to enhance antibacterial effect
What do we do after placing a cotton wool pledget into the canal?
We place out temporary restorative material which is usually ChemFil
What is chemFil?
Zinc Reinforced Glass Ionomer Cement
What are the benefits of ChemFil (Zinc Reinforced Glass Ionomer Cement)
- It is self curing so can be placed in on increment
- Sets quickly so works fast
- Packable
- Strong
- Good radio opacity and contrast what shade available for easy removal
