Patient Centred vs. Person Centred Care
Patient = disease perspective
Person = about the person/their life, experience
What is the shifting perspectives model of chronic illness?
How the patients perceives themselves … ill or well? Shifts with time
What are the 6 themes of patient centred care?

- Start from the patients situation
- Legitimize the illness experience
- Acknowledge the patient’s expertise
- Develop partnership
- Offer realistic hope
- Provide patient advocacy
What types of barriers can there be to patient-centred care?

What is partnership?
A relationship and process between provider & patient … find a common ground.
What is shared decision making?
What are its elements?
A partnership between provider & patient where there is bidirectional exchanges leading to a shared decision.
What is a transition?
A passage from one life phase to another.
Patient perception is most important.
What are the principles for practice?
- Chronic illness affects all dimensions of personhood.
- Cultural responses to illness are important.
- Incorporate a team approach.
- Adopt a ‘whole of life’ approach (risk factors)
- Care that is person-centred and family inclusive
How does multiple sclerosis affect someone?
- challenges getting diagnosis
- stabilizing symptoms
Differentiate acute care models vs. chronic care models
Acute: disease & doctor centred, individual & cure focus
Chronic: person & team centred, population health approach & management focus
What is the illness trajectory model?
How is it used?
It has illness phases ranging from prevention to dying.
Each phase requires different goals for care and treatment.
What is resiliency?
Why is it important to chronic illness?
The ability to bounce back from adversity.
Often determines quality of life with chronic illness.
