When should palliative care be introduced?
At diagnosis of a life threatening illness preferably
What does palliative care aim to help with?
- prepare for and manage end of life choices
- cope with loss and grief
- treat and prevent active & new issues
- adress expectations, needs, hopes, fears
What does palliative care focus on?
Focus on how well a person lives rather than how long they live
What is the Palliative Performance Scale (PPS)?
A measure of the patients level of functioning using %
30% or below is end of life
What are the 3 phases of palliative care?
Early
Advanced
Terminal
Palliative care vs. end of life care
What is the Gold Standard Framework?
Identify people who may need additional supportive care as they near the end of life… when to initiate palliatve care.
Ask “Would you be suprised if this patient died in the next 6-12 months?”
