Week 13 Flashcards
What is an example of an appropriate strategy to utilize in the development/delivery of a home exercise program for geriatric individuals?
after the individual reads and demonstrates the exercises, say “please share your questions with me”
describe the concept “slippery slope of aging”
individual performance (fun, function, frailty, failure) can decline or improve with lifestyle and health conditions that influence physiologic capacity
a 75 year old patient is evaluated by home health physical therapy for history of two falls, one fall occurred at night while walking to the bathroom. The other fall occurred while descending stairs during the day. The patient wears glasses and hearing aides. What is the best environment modification for fall risk management?
nightlight with red bulb in sleep area
A 78-year-old man is 3 months status post right total hip arthroplasty (THA) for osteoarthritis. Comorbid health: HTN (takes lisinopril), presbycusis (wears hearing aides).
Functional history: Independent with BADL/ IADL and community ambulation without assistive device until 6 months prior to THA, then used left single point cane (SPC) and family assisted with ADL. After THA, patient gradually progressed ambulation endurance using a rolling walker and discharged from home health PT once able to ambulate with SPC. Patient discusses his goal to walk with spouse and dog on the beach 3 times/week with his geriatrician.
Which of these approaches is MOST consistent with geriatric best practice principles for this scenario?
Refer to outpatient PT for evaluation of gait, balance, strength and endurance to determine prognosis and plan for
community mobility on level and unlevel surfaces without assistive device.
Calculate the maximal heart rate for an 80-year-old individual using formula MOST appropriate for geriatric population. (Note: this is step 1 in process of calculating target heart rate for exercise).
152 bpm
What is an appropriate communication approach to patient education with older adults.
Encourage the patient to demonstrate: “Show me how you will do that exercise at home.”
An 82-year-old patient comments that “feeling sad and worried is expected with getting older.” How should the health provider interpret this comment?
Concern that expression of internal ageism may be linked to increase in chronic health conditions.
A 77-year-old individual BOI (born-on-the-island) has lived in their beach house for 45 years. Walking on the beach has become more difficult following a stroke with mild left side weakness. Which of these strategies BEST reflects an asset based aging approach?
Walking on the beach with adaptive equipment and family or friends.
What are the Geriatric Ms?
(5 Ms)
1) medications: polypharmacy, optimal prescribing, deprescribing, side effects
2) mobility: frailty, falls, function
3) mind: depression, delirium, dementia, encephalopathy
4) multicomplexity: multi morbidity, geriatric syndromes, psychosocial needs
5) matters most: caregiver support, advance care planning, goals of care
List the guiding principles of geriatric PT
1) inter professional: collaborative practice that is inclusive of patients and their caregivers
2) person centered care: to elicit and prioritize the individual’s preferences, values, and goals to drive the POC
3) physical activity: to promote well being, chronic disease management, and enhance mobility
4) evidence informed: provide positive outcomes of PT care by completing interventions based on the best available evidence
5) holistic assessment: utilizes sound outcome measures to inform the treatment plan and relate it to pt’s stated goals
6) anti ageist: strive for anti ageist practice
As a PT, do we dive deep into each of the 5 Ms?
no, some of the Ms will be the PT screening it. (Example: medications)
Whereas mobility is our specialty
