Week 11 Flashcards
What are the three components that blood products are separated into by centrifugation
Plt
RBC
PLASMA
What are the different components that can be derived from blood products
- Red Blood Cells (RBC) - can be further modified
- Plasma (FP) - must be frozen within 24 hours
- Plasma (AFFP) - must be frozen within 8 hours
- Cryosupernatant Plasma (CSP)
- Cryoprecipitate
- Platelets
- Plasma Fractionation Products - IVIG
- Protein - Albumin 5% or Albumin 25%
- Coagulation factors, which can be a human plasma source or recombinant, are produced
What is the universal leukocyte reduction method
-pre storage leukoreduction method
-leuks are removed from RBC and platelet portion before storage
* Pre-storage leukoreduction to a threshold of 5 X 106 per component can prevent most FNHTR
associated with RBC and platelet component transfusions
What is the circular of information
- Extension of the Component label
-states info on packaging , storage, and handling
-warnings and precautions
-dose and information
-volumes and counts per bag
how are Red Cell products stored
- Leukocyte Reduced (LR)
- SAGM (Additive) is added
- Expiry date is 42 days if the product is not modified
- Approximately 480 mL of whole blood collected with 70 mL of CPD anticoagulant
- RBC LR SAGM stored at 1 -6̊ C
- Refer to the Circular of information for Detailed product information
What are the different types of plasma products
AFFP
FP
Octaplasma
cyroprecipitate
cryosupernatant plasma
how is AFFP collected
-by apheresis and frozen in 8 hours of collection (have FV, FVIII and non labile coagulation factors)
how is FP collected
-separated from whole blood and frozen in 24 hours of collection (low FV and FVIII)
How is octaplasma collected
-solvent detergent treated plasma
-pooled fresh plasma is filtered so cells and debris are removed helps to reduce transfusion related lung injury
what is cyroprecipitate
-has only the proteins that precipitate out after you freeze plasma during slow and cool thaw
What is cryosupernatant plasma
has everything except the proteins that end up in the cyroprecipitate
stored at -18 or colder for a maximum period of 12 months
What are two methods of producing plasma fractionation products
plasma derived (PD) from Cohn or Cold ethanol fractionation
-pooled plasma from female donors
Recombinant (R) uses biotechnology
-cultured cells produce and secrete proteins into a medium = collected and purified
What is the whole blood without platelet retrieval method
IF only RBC and plasma were needed
-cool blood down to 1-6
-filter
-HARD SPIN
-remove plasma
-add SAGM to RBC
-process plasma
use CPD and SAGM
What is the Buffy Coat Production Method
(BCPM)
IF platelets are needed
-plts need to be seperated from WB in 24 hours
-cool WB with rapid cooling at 18-24
-hard spin
-separate WB into 3 layers BC, Plasma and RBC
-leave only BC in bag
-Add SAGM to RBC and filter
-freeze plasma as FP or make into cyroprecipitate
-BC has WBC and PLT
-BC from 7 people and plasma from 1 pooled
-SOFT spin PRP separated and filtered to use for plts
-wbc discarded
use CPD and SAGM
What does the label for blood collection bag contain
- Donor Center will label all units
- ABO/Rh
- Collection and Expiry Date
- Modifications and Volumes
- Special Typing, e.g., phenotyping
- Unit identifier
- Product Type
