two main enzymes that work on arachadonic acid and what they can make from it
lipoxygenase: leukotrienes
Cyclooxygenase: prostaglandins, thromboxane, prostacyclin.
Prostaglandins
vasodilation,
gastic cytoprotection,
nociceptor sensitization!
Thromboxanes
platelet aggregation!, inflammation
Prostacyclin
vasodilation
gastric cytoprotection
inhibits platelet aggregation!
COX 1 vs COX 2
1: NORMAL PHYSIOLOGY gastric cytoprotection, vasodilation, platelet aggregation
2: INDUCABLE acute inflammation
prostaglandin involved in fever
PGE2
Adverse effects of high dose aspirin
Respiratory alkalosis - hyperventilation increases blood pH
GI bleed
if dose is at toxin levels, aspirin can uncouple oxidative phosphorylation (primarily in skeletal muscle) lactic acid accumulates in serum, decreases blood pH.
dehydration, agitation, etc.
Tx of aspirin overdose
induce vomitin, activated charcoal, give sodium bicarb if acidotic
can need transfusion, dialysis
Cause of aspirin hypersensitivity
treat with
build up of A.A. causes influx into leukotriene pathway, bronchial constriciton
Tx: epi
Reye’s syndrome
aspirin use in presence of viral infection
naproxen general
similar to ibuprofen, longer acting
