WBC: Hodgkin's Lymphomas Flashcards
What are the Lab Findings in HL?
-
ESR elevated
- >70mm in 1st hr = unfavorable prognosis
-
LDH elevated
- Correlate with the bulk of the ds (tumor burden)
-
ALP may be increased
- Due to liver or bone involvement
What are Reed-Sternberg Cells?
Binucleated, or multinucleated, Giant Cells with eosinophilic inclusion-like nucleoli
What is Nodular Sclerosis HL subtype characterized by?
- Nodular division of affected lymph nodes by fibrous bands
- Presence of lacunar cells, Reed-Sternberg cell variants
Who is Nodular Sclerosis HL most common in?
- Younger pts
- Developing countries
Where does Nodular Sclerosis often arise?
- Upper mediastinum
- Lower cervical nodes
- Supraclavicular nodes
What is Mixed Cellularity HL subtype characterized by?
- Polymorphic infiltrate of eosinophils, plasma cells, histiocytes, and Reed-Sternberg cells
- Areas of necrosis and fibrosis
Who does Mixed Cellularity HL most commonly affect?
- Middle-aged pts
- Most common among HIV-infected pts
What is the Lymphocyte Depleted HL subtype characterized by?
- Few lymphocytes
- Numerous Reed-Sternberg Cells
- Areas of Necrosis and FIbrosis
Who does the Lymphocyte Depleted HL subtype mainly occur in?
Elderly
Which HL subtype indicates extensive ds and has the poorest prognosis?
Lymphocyte Depleted
What does the Lymphocyte Depleted HL subtype have the propensity to involve?
- Retroperitoneal lymph nodes
- Extralymphatic sites
Which HL subtype is the most difficult to distinguish from NHL?
Lymphocyte Depleted
What is the Lymphocyte Predominant HL subtype characterized by?
- Large numbers of lymphocytes and histiocytes
- Differ from the Reed-Sternberg cells found in the other subtypes
- Paucity of Reed-Sternberg cells
Who are typically the pts with the Lymphocyte Predominant HL subtype?
Young men, often with no symptoms
Which HL subtype is slow growing and has the best prognosis?
Lymphocyte Predominant
