Neutrophil w/ single or bilobed nucleus (Pince-Nez or Eyeglass, Peanut, Dumbell shaped)
Benign
Inherited: Pelger-Huet Anomaly
Heterozygous: Bilobed
Homozygous: Round
Acquired: Pseudo-Pelger-Huet Anomaly or Pelgeroid
Hyposegmented Neutrophil
-Neutrophil with 6 or more lobed nucleus
-Abnormality in DNA synthesis
-So-called Macropolycytes
Inherited: Undritz Anomaly
Acquired: Megaloblastic Anemia
Hypersegmented Neutrophil
-Represents the second X chromosme in females not found in males
-Round projections of nuclear chromatin connected to the nucleus of the neutrophil by a single, fine strand of chromatin
-Referred to as Drumstick
Barr Body
-Composition: Fused Primary Granules
-Pink or red rod-shaped structures found in cytoplasm of myeloblasts, monoblasts and promyelocytes
Found in: M3, DIC, AML
Auer Rods
Faggot cells = cell with mass of Auer Rods
-Compositions: Free ribosomes of Rough ER
-Single or multiple light gray-blue oval inclusions in the cytoplasm of neutrophil and eosinophils found near the periphery of cell
-May resemble May-Hegglin Inclusions
Found in: Severe infxn or Toxic States
Dohle Bodies
-Composition: Primary granules showing increased ALP activity
-Large purple to black granules or dark blue-black granules in the cytoplasm of neutrophils
Found in: Severe infxn or toxic states
Toxic Granules
-Compositions: Open spaces from phagocytosis
-Represent the end stage of phagocytosis
-Clear, unstained circular
Found in: Severe infxn, Toxic States
Toxic Vacuoles
-Composition: Stored Mucopolysaccharides
-Large purplish granules in the cytoplasm of all leukocytes
-May resemble toxic granules
Found in: Alder-Reilly anomaly, Hurler’s & Hunter’s Syndrome
Alder-Reilly Granules
-Large dark granules in neutrophils (gray-green), monocytes (Purple), and lymphocytes (red) and giant lysosomes found in the cytoplasm
-Represents abnormal or defective lysosome
Found in: Chediak-HIgashi syndrome
Chediak-Higashi Granules
-Larger than usual Dohle-like inclusion bodies in neutrophils
-Rare anomaly affecting neutrophils and platelets
Found: May-Hegglin Anomaly
May-Hegglin Granules
-Phagocytes can ingest but cannot kill catalase-positive organisms due to defective or impaired respiratory burst oxidase system
Test: NBT dye test
Chronic Granulomatous Disease
-Lymphocyte with round mass of chromatin “Ballerina Skirt” appearance
-React with B cells infected by EBV
Found: Infectous mononucleiosis aka Downey cells
Atypical Lymphocytes
-Degenerated nucleus
-Ruptured cell in form of smudge or basket
-Fragile lymphocyte and break upon smearing
Found: CLL
Basket cells/ Smudge cells
-B-cell with hair-like cytoplasmic projections
-Found in Hairy cell leukemia
Hairy cell
-T-cell with brain-like convolutions in the nucleus
Found in: Sezary
Sezary Cell
