Cyst divide to trophozoites. Trophozoites multiply and produce cysts but can also invade tissue

Sx
Recurrent abdominal cramping and diarrhea
Fever, chills, bloody or mucous diarrhea and weight loss
Small volume diarrhea
Tenesmus (straining and cramping)

Fever and right upper quadrant pain
Liver abscess

Liver abscesses
Don’t occur with bowel symptoms, ameba are found in stool less than 20% of the time

Amebic pulmonary disease
Liver abscess ruptures through diaphragm
Pleuritic pain, RUQ pain, cough, expectoration, SOB

Diagnosis
E. Histolytica, mashkovskii and dispar all look the same!

Stool for O&P (w/in 30 min)
Gliding motion with pseudopods
See multiple mucosal ulcers
Flask shaped ulcers

Nuclei in cysts and trophozoites
Cysts: 4
Trophozoites: 1
Also Charcot-Leyden crystals
E. Coli vs E. histolytica
How to tell
Nuclear chromatin is fine and dispersed in histolytica : sharp and central karyosome
Coarse and clumped for e. Coli: eccentric Karyosome
E: coli has 5 or more nuclei

Metronidazole or tinidazole

Largest intestinal protozoan
Only ciliate infxn which causes disease in the intestine of humans

Cryptosporidium parvum
Transmission
