A

*present similar to bowel obstruction

  • colicky pain - occurs after few hours
  • abdominal distention
  • Constipation
  • vomiting (late sign in sigmoid volvulus
  • Tympanic precussion

  • CT c contrast abdo-pelvis - initial Ix
    • much more sensitive, specific and identifies location and cause
    • whirl sign

A
  • sigmoidoscope decompression
  • insertion of flatus tube
    • left for up to 24 hrs
    • after decompression

What are the differentiating features between sigmoid and caecal volvulus on AXR?

A

Sigmoid

  • arises in the pelvis (left lower quadrant)
  • extends towards the right upper quadrant
  • ahaustral in appearance
  • sigmoid volvulus causes obstruction of the large bowel, therefore the ascending, transverse and descending colon may be dilated

Caecum

  • arises in the right lower quadrant
  • extends towards the epigastrium or left upper quadrant
  • colonic haustral pattern is maintained
  • distal colon is usually collapsed and the small bowel is distended
