Hand-sanitizer

Greet patient and introduce self, identify patient

Note if patient is wearing glasses or contacts

Explain procedure to patient, read each line that is being pointed to and to keep both eyes open at all times.

Tell patient where they are to stand (20 ft away) and ask patient to read with both eyes open

Cover left eye with an occluder and tell them to read and record smallest line they are able to read.

Do the right eye and record smallest line they are able to read and record any accommodations they needed to take to read.

Tell patient to sit up straight but comfortably in a chair and give the jaegar chart and tell them to hold it about 14-16 inches away from the eyes.

Tell patient to read with both eyes open, first with glasses/contacts and then without

Record results and if any also problems on patients chart.

Thank the patient and ask patient if they have any questions.

Cleaned supplies and returned them to proper storage and use hand-sanitizer
