Dengue Fever:
A
- Flavivirus
- Aedes aegypti (inverted lyre on back) and Aedes albopictus (white stripes on back)
- Infects Monocytes and Endothelium
- Fever, arthralgia (break bone pain), erythematous rash, petechiae (leaky blood vessels)
- Decrease Hct and Platelets
- Detect by RT-PCR for Viral RNA and ELISA to detect IgM

Dengue Hemorrhagic Fever and Dengue Shock Syndrome:
A
- Occurs with your second infection of Dengue Fever but by another serotype
- Bleeding at skin, nose and GI tract
- Shock due to plasma loss
- Antibodies from first infection bind the second virus but does not work since it is a different serotype since it infects monocytes the antibodies actually make it easier for the virus to infect monocytes when they phagocytose them

Parvovirus B19:
A
- ssDNA
- Infects HSC in bone marrow
- Erythema infectiosum in children -> Slapped Cheek
- Lace-like reticular rash in adults
- Infects joints
- Hyrdrops Fetalis -> cross placenta and causes profound anemia
- May have anti-DNA antibodies
- If you have more than one Arthritis in the a household it is probly Parvovirus B19

Hepatitis B:
A
-Only develop arthritis with Hepatitis B if you have detectable HbsAg and anti-HBs immune complexes in joints

Hepatitis C:
A
- Cryoglobulins seen in HCV
- Immune complexes deposit in vascular wall activating complements and causing leukocytoclastic vasculitis -> Palpable purpura
- HCV infects B-cells over stimulating them creating RF
- Liver will typically clear the RF cryoglobulins but not in HCV since the liver is obviously infected in hepatitis
- Treat with Harvoni

Rubella Virus and Rubella Vaccine:
A
- Togavirus
- Rash, fever, lymphadenopathy
- Directly infects synovial tissues
- Not immune complex driven
- Diagnose with antiviral IgM
- Self-limiting

Lyme Disease:
A
- Borrelia burgdorferi
- Ixodes scapularis carries it
- Early disseminated disease -> Fatigue, Bells Palsy, AV Block
