prescription requirement for POM-V and POM-VPS (9)

A
  1. prescriber - name, telephone, address, qualification, signature (if CD2/3 then RCVS number)
  2. Owner - name and address
  3. identification of animal, species - address if different to owner
  4. Date - valid for 6 months, CD2/3/4 = 28 days
  5. Medicine - name, quantity, dose, administration instructions
  6. warnings - withdrawal period
  7. if under cascade then ‘under vet cascade’
  8. repeatable - number of times it can be repeated
  9. Schedule 2/3 wording ‘item prescribed for an animal/herd under care of vet’
