A
retain 5 years or just keep all records of documents in book for 5 years

prescription requirement for POM-V and POM-VPS (9)
A
- prescriber - name, telephone, address, qualification, signature (if CD2/3 then RCVS number)
- Owner - name and address
- identification of animal, species - address if different to owner
- Date - valid for 6 months, CD2/3/4 = 28 days
- Medicine - name, quantity, dose, administration instructions
- warnings - withdrawal period
- if under cascade then ‘under vet cascade’
- repeatable - number of times it can be repeated
- Schedule 2/3 wording ‘item prescribed for an animal/herd under care of vet’
