Do requisitions for CDs in hospitals requested by the same entity in the hospital need to be on a approved form?
No, not same entity. But they do if its being requested OUT of the hospital.
When can you supply emergency requisition request from a GP without the requisition?
If requisition for CD 2 or 3 is agreed to be supplied after 24 hours. Failure, is an offence on behalf of GP.
How do we process CD requisitions?
Submit to NHS agency and retain a copy for 2 years from date of supply
What are the prescription requirements for controlled drugs?
- Signature of prescriber
- Address of prescriber - SHOULD be in UK for CD2+3
- Dose ‘as directed’ NOT acceptable
- Strength - if more than one available
- Formulation
- Quantity - total should be in words AND figures
- Instalment wording where appropriate
- Dental wording where appropriate
- Patient address AND name
- Date - 28 day validity and should only be prescribed amount for 30 days

Private prescription requirements for CD2 and 3?
- use designated standard forms
- prescriber IDENTIFICATION number not GMC
- submit to NHSBSA UNLESS vet rx then retain for 5 years
- dont prescribe POMs on private aswell as it needs to be retained for 2 years
Which schedule 3 drugs are exempt from safe custody?
Gabapentin, Pregabalin, Phenobarbital, Midazolam, Tramadol
