what is Vestibular neuronitis?

A

Vestibular neuronitis is thought to be due to inflammation of the vestibular nerve and often occurs after a viral infection.

how does Vestibular neuronitis present?

A

Spontaneous onset of vertigo,
commonly occurs on first awakening (2)
the patient may feel very unwell and they often lie still in bed

nausea, vomiting,
unsteadiness

Hearing loss and tinnitus are NOT present

NO focal neurological symptoms

Although the severe initial symptoms usually last 2–3 days, people with vestibular neuronitis usually recover gradually over a period of weeks through a process of central nervous system compensation.

A

Presence of nystagmus - usually fine horizontal, fast phase away from the affected ear. It always beats in the same direction,

RX of VN?

A

Advice should be offered regarding resuming activity as soon as possible, and safety issues such as driving, work, and prevention of falls.

If symptoms are severe, short-term symptomatic drug treatment can be offered:

  1. Buccal or intramuscular prochlorperazine or
    1b. IM cyclizine

can be considered to rapidly relieve severe nausea or vomiting associated with vertigo.
