A
  1. Carry out LOC history to establish before, during, after

2. Ask about red flags (known heart defects, occurred during exertion)

What Ix are done and why?

A
  1. Bloods – capillary blood glucose, FBC, U&Es
  2. ECG – check for arrhythmias
    ?echocardiogram if structural heart defect suspected (AS)
    ?CT/MRI if epilepsy suspected

A

Excessive vagal discharge causes drop in blood flow to brain - Due to drop in BP & HR

What are signs/symptoms in vasovagal syncope?

A
  1. Sweating/dizzy/light-headed before LOC
  2. Limb twitching
  3. Nausea
  4. Pallor

What Ix are done for vasovagal syncope?

A
  • As before (ECG, bloods, imaging) +
  • Consider tilt-table test / carotid sinus massage
  • bHCG

What is the management for vasovagal syncope?

A
  • Patient education & avoid triggers

- Physical techniques (arm tense, leg cross)

What is a carotid sinus massage?

A
  1. apply pressure to carotid sinus massage for 5 sec

2. If decrease in SBP >50mmHg or ventricular pause > 3secs = positive result
