Blockage in electrical signal from AV node
What are different heart block?
- 1st degree
- 2nd degree:
- Type I / Mobitz I / Wenckebach
- Type II / Mobitz II - 3rd degree / Complete Heart Block
What are symptoms for type 1 and mobitz I?
asymptomatic
What are symptoms of Type 3 and mobitz II?
- Chest pain
- Palpitations
SOB - Syncope
What are causes of heart block?
- Post-MI / ACS
- Drugs – beta-blockers, CCBs
- Metabolic imbalances – ↑K+
- Hypertension
What is a high degree AV block and syncope?
Stokes-Adams Attack
What are the ECG changes in 1st degree?
prolonged PR Interval >0.2s
What are the ECG changes in Mobitz I: Wenckebach?
PR interval gets longer and longer then QRS dropped
What are the ECG changes in Mobitz II?
QRS complex dropped at regular ratio
What are the ECG changes in complete heart block?
No association between O waves and QRS complex
What investigations are done for heart block?
- ECG
- Troponin
- K+, Ca2+, pH
- Serum digoxin levels
What are complications for heart block?
- Heart failure
- Sudden cardiac death
- Pacemaker complications
What is the management of Asymptomatic + 1st degree/Mobitz I?
monitor
What is the management of Symptomatic + 1st degree/Mobitz I?
discontinue AV-node blockers + ?pacemaker/CRT/ICD
What is the management Asymptomatic/slightly symptomatic + Mobitz II / Complete HB?
discontinue AV blockers + pacemaker/CRT/ICD
