Pain brought about by walking and relieved by rest
Intermittent claudication due to arteriosclerotic occlusive disease (no work up if life is not interfered)
Work up of disabling intermittent claudication
- Doppler studies (look for pressure gradient)
Neg doppler studies for intermittent claudication indicates
the disease is in the small vessels and not amenable to surgery
Doppler studies show significant pressure gradient NSIM?
- CT angio or
- MRI angio
Short stenotic segments of lower extremities arteriosclerotic occlusive disease is treated by
angioplasty and stenting
Long and more extensive stenotic segments of lower extremities arteriosclerotic occlusive disease is treated by
bypass grafts
sequential stents
longer stents
Grafts originating at the aorta (aortobifemoral) are done with
prosthetic material
Bypass between more distal vessels (femoropopliteal or beyond) are usually done with
reversed saphenous vein grafts
Tx for incomplete arterial emoblization occlusion
clot busters
Tx for complete arterial emoblization occlusion
fogarty catheter
and fasciotomy if several hours have passed before revascularization
