Berry Aneurysms most commonly seen where
branching of ACA
Rupture of berry aneurysm causes
Subarachnoid hemorrhage….pt complains of worst headache ever
What is xanthochromic spinal tap?
Means that the blood has been there a while and has been turned into bilirubin
What is there a risk of 2-3 days after the aneurysm
vasospam
How do you treat the potential vasospasm from a SAH?
Nimodipine…..remember “dipine” means dihydropyridine Calcium Channel Blocker which prevents smooth muscle contraction in vasculature.
DRUG AFTER SAH
NIMODIPINE
Epidural hematoma
FAST BLEED
usually due to rupture of middle meningeal artery.
Often secondary to bone fracture.
Pts will havea lucid interval where they seem fine
Forms a lens shape as it peels the dura off the skull
SUB-dural hematom
SLOW BLEED….seen in whiplash
Crescent shape due to spreading
