What is vasa praevia?
When a foetal blood vessel runs in the membranes in front of the presenting part (between the foetal head and the internal os)
Why does vasa praevia usually occur?
Because the umbilical cord is attached to the membranes rather than the placenta (velamentous insertion) or where the placenta is in parts
Diagnosing vasa praevia?
Very difficult, often not able to do it on USS
How can vasa praevia cause APH?
If the vessel ruptures
When is vasa praevia rupture more likely to occur?
When the vessels are closer to the cervix (i.e. praevia)
What happens when the vasa praevia ruptures?
Massive foetal bleeding
Presentation of ruptured vasa praevia?
Painless, moderate bleeding ay the rupture of membranes
What is the problem with vasa praevia rupture?
Severe foetal distress follows
Usual delivery with vasa praevia?
C-section, though this is often not fast enough to save the foetus
