Q

Mitral regurgitation treatment?

A

Observe in mild cases, watch for thrombus
Medical treatment – diuresis, rhythm control/anticoag
Surgical - MV repair, replacement is you have to

10
Q

Concern with calcific aortic stenosis?

A

May affect the conduction system if Ca extends to the conduction system

11
Q

Pathology of Aortic Stenosis

A

Increased afterload with impaired LV emptying in systole
Concentric LV hypertrophy, diminishing compliance
Atrial systole becomes more and more important
LV hypertrophy can eventually cause LV failure
Hypertrophy increases blood requirements for ventricle

16
Q

EKG findings with aortic stenosis

A

LVH

May have IVC delays like R and L BBB, AV nodal block, atrial fib

17
Q

CXR findings with aortic stenosis?

A

Calcification of the aortic valve/aorta
LVH
Post stenotic aorta enlargement

18
Q

Echo measurement done in aortia stenosis

A

measure AVA (0.8 or less is severe)

19
Q

Medical therapy for aortic stenosis?

A
Control hypertension (be careful with diuretics)
Control arrythmias
20
Q

What to do for a high risk patient who needs replacement but is too high risk

A

Transcatheter Aortic Valve Replacement

21
Q

Causes of aortic regurgitation?

A

Calcific AV disease, Congenital, Rheumatic
Endocarditis, aortic dissection,
Annuloaortic ectasia, RA, VSD, Myxoma

22
Q

What is annuloaortic ectasia?

A

abnormal dilation of the annulous and aortic root

Associated with Marfans

23
Q

Changes of the heart in aortic regurgitation?

A

Eccentric hypertrophy of the LV
(Increased chamber size AND increased wall thickness)
Can cause massive dilation of LV - Cor Bovinum

24
Q

Symptoms associated with aortic regurgitation?

A

Dyspnea, Orthopnea, PND, Angina

Rarely – Syncope

25
Q

Describe an aortic regurg murmur

A

Diastolic blowing murmur
Austin Flint Murmur – Regurgitant jet hits the anterior leaflet of MV, closes the anterior valve and moves murmur to apex

26
Q

Aortic Regurgitation CXR findings

A

LV enlargement
enlarged Ascending Aorta
Pulmonary Edema
Increased LA

27
Q

EKG findings in Aortic Regurgitation?

A

LVH

28
Q

Echo findings in Aortic Regurgitation?

A

Regurgitant jet

29
Q

Surgical procedure typically recommended for aortic regurg?

A

Valve replacement

May have to replace ascending aorta depending on etiology

30
Q

What should lead you to recommend a bioprosthetic valve?

A

Women of child bearing age
Non-compliant patients
Older patients
People who can’t take blood thinners

31
Q

When should you recommend a mechanical valve?

A

Younger patients
