What is adenosis? What is its embryology and what is a risk factor?
Focal persistence of columnar epithelium in the upper vagina. During development, squamous epithelium from the lower 1/3 of the vagina grows upward to replace the columnar epithelium lining of the upper 2/3 of the vagina. Diethylstilbestrol (DES) exposure in utero.
Which structuresis the lower 1/3 of the vagina derived from?
Urogenital sinus
Which structure is the upper 2/3 of the vagina derived from?
Mullerian ducts
What is feared complication of DES associated vaginal adenosis? How does it look like on histology?
Clear cell adenocarcinoma which is a malignant proliferation of glands with clear cytoplasm
A 4 year old boy comes to your peds clinic with a complaint of bleeding . Upon inspection you observe a grape like mass protruding from the penis. What is causing this presentation? What is the mass called? You decide to stain the cells. What stains will you use? What are the cells called?
This is embryonal rhabdomyosarcoma which is a malignant mesenchymal proliferation of immature skeletal muscle. Can present in children less than 5 years old and the mass is called a sarcoma botryoides. Positive staining for desmin and myogenin of the rhabdomyoblast.
What can vaginal intraepithelial neoplasia (VIN) convert to? What is an etiology? Where does the cancer head to when it occurs in the regional lymph nodes?
Vaginal carcinoma which arises from squamous epithelium lining the vaginal mucosa. Usually related to high-risk HPV. Cancer from the lower 1/3 of vagina goes to inguinal nodes and cancer from upper 2/3 goes to regional iliac nodes.
