After you recognize v-fib rhythm you should defibrillate 1 shock at what setting of Joules?
- Biphasic: 120-200 joules
- Monophasic: 360 joules
What should you do before delivering the shock and what should you do after?
- Clear
- Resume CPR
After defibrillate and resume CPR how long do you continue CPR?
2 minutes/5 cycles (round 1)
What is the next step after completing the first round of 5 cycles of CPR?
Check rhythm –> is rhythm shockable? If yes administer another shock and continue CPR for 5 cycles (round 2)
After administering another shock what medication and dose should you administer during the 2nd round of CPR?
Administer IV Epinephrine 1mg
- repeat every 3-5 minutes
What should you consider after the second round of CPR?
Advanced airway and capnography
- 1 breath every 6 seconds (10 breaths per minute)
After the 2nd round of 5 cycles of CPR what should you check?
Check rhythm –> is rhythm shockable? If yes administer another shock and continue CPR for 5 cycles (round 3)
After administering another shock what medication and dose should you administer during the 3rd round of CPR?
Administer IV Amiodarone 300mg once and then consider an additional 150mg.
What other medication can you administer instead of amiodarone and what is the dose?
Lidocaine IV 1.0-1.5 mg/kg first then 0.5-0.75mg/kg. Max 3 mg/kg
What medication and dose should you administer for torsades de pointes?
Magnesium 1-2g IV
