Symptoms of UTI in baby
Fever, generalised abdominal pain, vomiting, dysuria, more wet nappies, increased frequency, incontinence, lethargy, irritability, poor feeding
Symptoms of UTI in older child
Suprapubic pain, incontinence, increased frequency, dysuria, vomiting
When is a MCUG used
Used to investigate atypical or recurrent UTIs in children under 6 months.
Also when FHx of reflux, dilation of ureter on US or poor urinary flow
Diagnosis of pyelonephritis
Fever of >38 degrees, loin pain or tenderness, UTI picture
Risk factors for pyelonephritis
Poor urinary flow, recurrent UTIs, recurrent fever of unknown origin, antenatal kidney problems, VUR or renal disease, constipation, dysfunctional voiding, enlarged bladder, spinal lesion, poor growth, HTN
What is the best investigation for children under 2 to diagnose UTI
Microscopy rather than urine dip
When to give US scans for UTIs
Children under 6 months, children with recurrent UTIs and atypical UTIs
How long to give course of ABx if suspected cystitis
3-5 days
How long to give course of ABx if suspected pyelonephritis
7-10 days
When is a DMSA scan used
3-6 months after illness to assess for damage from recurrent or atypical UTIs
