What to see in UTI
dysuria, frequency, WBC but NOT WBC casts in urine
UTI pathologic always
Males: infants with congenital defects, vesicoureteral reflux. Elderly with enlarged prostate.
Pyelonephritis
Fever,chills, flank pain, CVA tenderness, hematuria AND WBC casts
More common in
Women (shorter urethra colonized by fecal flora)
Other predisposing factors: obstruction, kidney surgery, cath, GU malformation, DM, and pregnancy
Dx markers
positive leukoesterase = bacterial UTI
positive nitrate test = gram negative bacterial UTI
positive urease test = proteus, klebisella
negative urease test = e coli, enterococcus
E coli
1st leading cause of UTI, shows green metallic sheen on EMB agar
S. sapophyticus
2nd leading cause of community acquired UTI in sexually active women
Klebsiella pneumo
3rd leading cause of UTI
Large mucoid capsule and viscous colonies
Serratia mascescens
Some stains produce red pigment
Often nosocomial and drug resistant
Enterobacter cloace
Often nosocoial and drug resistant
Proteus mirabilis
Motility causes swarming on agar
Produces urease
Associated with struvite stones
Pseumonas
Blue green pigment and fruity ordor
Usually nosocomial and drug resistant
