Systemic mycoses
all can cause pneumo, and disseminate.
All caused by dimorphic, except for coccidio, which is a spherule (not yeast) in tissue
Treat with fluconazole or itraconzole for local infection, but must use amphotericin B for systemic.
Systemic infection can mimic TB with granuloma fomration but no person to person transmission
Histoplasmosis
Mississipi and Ohio River Valley
Macropahges filled with histoplasma (smaller than RBC)
Histo hides in macrophages.
Birds or bat dropping
Blastomycosis
East of mississipi river and centra america
Form granumolatous nodules
Broad base budduing (same size as RBC)
“Blasto buds broadly”
Coccidio
Southwestern
Pneumonia and meningitis.
Spherules filled with endosmores, much larger than RBCs
Coccidio crowds
San Joaquine Valley or desert (desert bumps)
Paracoccidio
Latin America
Budding yeasts with Captain’s wheel formation (much larger than RBC)
Captain wheels appearance
