BPH Treatment Meds
Alpha-1 blockers- 1st line
5 alpha-reductase inhibitors
Combo: Dutasteride-Tamulosin (Jalyn)
Tadalafil (Cialis)

Terazosin (Hytrin) Doxazosin (Cardura) Alfuzosin (Uroxatrol)* Tamsulosin (Flomax)* Silodosin (Rapaflo)* used for BPH SYMPTOM MNGMT
Alpha-1 blockers Side Effects
Most common are dizziness and orthostatic hypotension
Terazosin (Hytrin) and Doxazosin (Cardura) (cause more BP lowering than the others)
problems with ejactulation
DONT use with ED meds (phosphodiesterase-5 inhibitors)
5 alpha-reductase inhibitors
Finasteride (Proscar)
Dutasteride (Avodart)
that provide long term decrease in prostate size and decreased need for prostatic surgery
5 alpha-reductase inhibitors Side Effects
Decreased libido
Ejaculatory or erectile problems
Decreases serum PSA by 50%
Erectile Dysfunction Meds
Prostaglandin injectable: Alprostadil Phosphodiasterase inhibitors: Tadalafil (Cialis) Vardenafil (Levitra) Sildenafil (Viagra) Avanafil (Stendra)
Phosphodiasterase inhibitors Contraindications
Men taking nitrates
Caution with alpha-1 blockers due to risk for severe hypotension
Testosterone replacement
To treat low testosterone levels in primary or secondary hypogonadism
Primary – Testes fail to produce testosterone
Secondary – Pituitary or hypothalamus malfunction
Urinary tract analgesics
Phenazopyridine (Pyridium)
Used for relief of SE of UTI: burning, itching, increased frequency, etc.
Do not use if GFR
Long Term therapy for Interstitial Cystitis
Pentosan (Elmiron)
Treatment of urgency, urgency-predominant mixed or OAB symptoms: Muscarinic antagonists/Antispasmodics
Oxybutynin (Ditropan) (Ditropan XL) Tolterodine (Detrol) (Detrol LA) Trospium (Sanctura), (Trospium XR) Do not use in demented patients Contraindications for antimuscarinic agents, Gastric retention, Angle closure glaucoma
