What are the 12 steps of the 24 hr Cr Cl check list?

A
  1. Scan the Physicians order and previous order
  2. Fax order to Pharmacy if there are meds
  3. Check and double check patient’s name
  4. Phone lab to request the appropriate urine collection bottle for the test. Symbolize the PO – Dept Aware
  5. Transcribe the order onto the Kardex under Lab Tests (see example). Symbolize the PO – K x 2
  6. Complete the 2 requisitions - Specimens Other Than Blood and Lab Req Blood from the Physician’s order and include the following:

• Specimens Other Than Blood Req’n:

  • addressograph / label
  • write in addressograph box – Urine for 24 Hr. Cr. Cl.
  • room and bed#
  • ordered by
  • antibiotics yes/no
  • tick off box urine collection 24 hr
  • time/date to be done: start @ 0600 the next day, end @ 0600 the next day +1
  • complete weight and height
  • tick off box creatinine clearance
  • attach and addressograph a label with name of specimen and test

• Lab Blood Req’n:

  • addressograph / label
  • write in addressograph box – 24hr. Cr. Cl.
  • room and bed#
  • date/time to be done: the next day @ 1800 (middle of 24hr. time period)
  • diagnosis
  • ordered by
  • tick off the test
  • fax requisition to the Lab. Fax stamp the req.
  1. Symbolize the PO - RMO x 2
  2. Put requisition with label for urine sample in designated spot for RN to collect.
  3. Call Porter to pick up labelled urine collection bottle and requisition and deliver to Lab.
  4. Sign Off
  5. Stuff chart prn
  6. Flag chart for RN
