There are 11 steps of the Low Molecular Weight Heparin check list. What are they?
A
- Scan the Physicians order and previous order
- Fax order to Pharmacy if there are meds
- RN aware/Dept aware for any STAT’s
- Check and double check patient’s name
- Transcribe the order onto the Kardex/MAR
- Addressograph & complete the requisition from the Physician’s order and include the following:
- room and bed #
- date to be done
- time to be done
- diagnosis
- ordered by
- test name
- any miscellaneous information prn
- STAT” on requisition in RED INK prn
- Check patient Medication Profile/MAR to see if patient on any anticoagulation medications
- Attach labels/stickers prn
- Symbolize each lab order on the PO with K, RMO, MAR
(RN aware/Dept aware prn) - Put requisition in pick up slot for lab or send to lab via tube/fax
- Complete Daily Bloodwork Card prn
- Sign Off
- Stuff chart prn
- Flag chart for RN
