First-line for lower UTI
Contraindicated if sulfa allergy
ciprofloxacin
For lower UTI
For patients with sulfa allergy
nitrofurantoin (Macrobid)
For recurrent lower UTIs
phenazopyridine (Pyridium)
Indication: UTI pain relief
MOA: local analgesic effect
SE: well-tolerated, reddish-orange urine
mirabegron
Class: beta-3-adrenergic agonist/anti-spasmodic
MOA: selectively stimulates beta-3-adrenergic receptors—relaxing bladder smooth muscle
Indication: overactive bladder
SE: hypertension, urinary retention, UTI, headache
Given PO
oxybutynin
MOA: anti-cholinergic; inhibits actions of acetylcholine
Indication: overactive bladder, urinary incontinence
SE: dry mouth, constipation
Only given PO (can be ER)
