Orign from C5, sometimes also C4 anterior rami. Pierces middle scalene, descends deep to levator scapulae and rhomboids.

Supplies Rhomboids, occasionally levator scapulae
Dorsal Scapular n
Origin from anterior rami C5, C6, C7. Through cervicoaxillary canal, decends inferiorly on serratus anterior
Long Thoracic n
Supplies serratus anterior
Long Thoracic n
Origin from Superior Trunk, fibers from C5,C6,C4, passes laterally across lateral cervical region (posterior triangle), superior to brachial plexus, through scapular
Suprascapular n
Supplies Supra and Infra spinatus and glenohumeral joint
Suprascapular n
C5, C6 and C4, Superior Trunk, Descends posterior to clavicle and anterior to brachial plexus and subclavian artery - often gives an accessory root to Phrenic Nerve
Nerve to Subclavius
Supples subclavius and S.C. joint, accessory phrenic root supplies diaphragm
Nerve to Subclavius
Lateral Cord, C5, C6, C7. Pierces Costocoracoid membrane to reach deep surface of pectoral muscles
Lateral pectoral n
Supples Pectoralis major - some lateral bracnhes to pectoralis minor via medial pectoral nerve.
Lateral pectoral n
Terminal branch of Lateral cord, C5-C7. Exits axilla by piercing coracobrachiallis, decends between biceps brachii and brachialis - continues as lateral cutaneous nerve of forearm
Musculocutaneous n
Supplies muscles of anterior compartment of arm - coricobrachialis, biceps brachii, brachialis, skin of lateral forearm
Musculocutaneous n
Formed from lateral and Medial roots from lateral and medial cords respectively., C6,C7 and C8,T1. Originates lateral to Axillary artery. Decends through arm adjacent to brachial artery, crossing anteriorly to become medial to the artery in the cubital fossa
Median n
Supplies muscles of the anterior forearm compartment - except FCU and the ulnar half of FDP. Also 5 intrinsic muscles on thenar side of the palm and palmar skin
Median n
Side branch of medial cord, C8,T1. Passes betwen axillary artery and vein then pierces pectoralis minor and enters deep surface of pectoralis major (actually runs lateral to lateral pectoral n but its name is derived from the cord it comes from)
Medial Pectoral n
Side branch of medial cord, C8,T1, Smalles nerve of plexus, runs along medialy to axillary and brachial veins. Communicates with intercostobracial n
Medial cutaneous n of arm
Supplies skin of medial side of arm
Medial cutaneous n of arm
Supplies pectoralis minor and Sternocostal part of Pectoralis major
Medial Pectoral nerve
Side branch of medial cord, C8, T1. Initially runs with ulnar nerve but pierces deep fascia with basilic vein, entering subcutaneous tissue, dividing into anterior and posterior branches
Medial cutaneous n of forearm
Supplies skin of medial side of the forearm
Medial cutaneious n of the forearm
Larger terminal branch of medial cord, C8, T1, often C7. Descends medial arm, passes posterior to medial epicondyl of humerus then descends medial side of forearm to the hand
Ulnar n
Supplies FCU and medial half of FPD. Most of the intrinsic muscles of the hand, skin of hand
Ulnar n
Supplies superior portion of subscapularis
Upper subscapular n
Side branch of posterior cord. C5, Passes posteriorly, directly entering subscapularis
Upper subscapular n
Supplies inferior portion of subscapularis and Teres Major
Lower subscapular n
Side branch of posterior cord, C6. Passes inferiorlaterally, deep to subscapular artery and vein
Lower subscapular n
Side branch of posterior cord, C6,C7,C8. Arises between upper and lower subscapular nerves. Runs inferiolaterally along poterior axillary wall to the apical part of latissimus dorsi
Thoracodorsal n
Supplies latissimus dorsi
Thoracodorsal n
Supplies ghelohumeral joint, teres minor and deltoid muscles, also skin of superolateral arm
Axillary n
Smaller terminal branch of posterior cord, C5, C6. exits axillary fossa posteriorly - passes through quadrangular space with posterior circumflex humeral artery - gives rise to superior lateral cutaneous n of arm then winds around surgical neck of humerus, deep to deltoid
Axillary n
Larger terminal branch of posterior cord (largest branch of plexus), C5-T1. Exits axillary fossa posterior to axillary artery. Passes posterior to humerousin radial grove with deep brachial artery between lateral and medial heads of triceps. Perforates lateral intermuscular septum - enters cubital fossa dividing into superficial and deep nerves which are cutaneous and motor respectively
Radial n
Supplies all muscles of posterior compartment of arm and forearm. Skin of posterior and inferolateral arm, posterior forearm and dorsum of hand.
