Upper GI Haemorrhage Flashcards
Important points to take from history
Timing - onset/length Colour Other vomit? Amount NSAID use Anti-coagulant use Alcohol
Emergency causes of upper GI bleeding
Oesphageal varices
Gastric ulcer
Non-emergency causes of upper GI bleeding
Mallory-Weiss tear
Oesphagitis
Dieulafoy lesion
What are oesphageal varices?
Dilatations of porto-systemic venous anastomoses. The dilated veins are thin so therefore rupture easily
Causes of oesphageal varices
Portal hypertension due to cirrhosis caused by alcoholic liver disease
Management of oesphageal varices
Band them
Inject adrenaline
Sengstaken-blakemore tube
Porto-systemic shunt
What is gastric ulceration?
Erosion of blood vessels by ulcer
Management of gastric ulcers
Gastroscopy PPI therapy Ulcer biopsy Adrenaline injection Cauterisation
What is a Mallory-Weiss tear?
Tear in oesphageal epithelium, caused by repeated vomiting.
Most cases resolve themselves
What is oesophagitis?
Inflammation of intraluminal epithelial layer, caused by GORD or Crohn’s
What are Dieulafoy lesions?
Abnormally large veins - vascular malformation
Can cause bleeding
What is used for prophylaxis of oesophageal bleeding?
Non-cardio selective B-blocker (propranolol)
Causes vasoconstriction
