How may bleeds present?
- Stained sputum

- Haematemesis
- Melaena
What score can grade an upper GI bleed?
Glasgow-Blatchford Score
What are the parameters of a Glasgow-Blatchford Score?
- Drop in Hb
- Rise in Urea
- Hypotension
- Tachycardia
- Melaena
- Syncope
What Glasgow-Blatchford Score indicates high risk?
Greater than 0
What score can be used post-endoscopy?
Rockall Score
How are Upper GI Bleeds treated?
ABATED
A-E approach Bloods Access (large bore cannulae) Transfuse Endoscopy (urgent 24 hours) Drugs (stop anti-coags and NSAIDs)
What bloods should be done and why?
- FBC (anaemia, plat)
- UsEs (Urea)
- Coag (INR)
- LFTs (Varices)
- XMatch (2 units)
What should you transfuse with in a big bleed?
Blood and FFP
What can excess blood transfusion cause?
TRALI (Transfusion Related Lung Injury)
How are varices treated?
- Terlipressin
- Prophylactic antibiotics
What is the definitive treatment in an UGI bleed?
OGD and intervention
What should be stopped prior to OGD?
PPI
