What is a paracetamol OD?
Greater than 4g in 24 hours
What may cause paracetamol OD?
- Accidental
- Self-harm
- Therapeutic
How does paracetamol OD present?
- N&V
- RUQ Pain
- Risk of self-harm, dementia etc.
May have: jaundice, hepatomegaly, asterixis
What blood test is used for paracetamol OD?
Salicylate level
What must always be checked with paracetamol OD?
- Alcohol
- Other drugs that may have been taken
What other bloods are done in paracetamol OD?
- Full profile with clotting
- ABG (lactic acidosis)
How is paracetamol OD treated?
- N-acetylcysteine within 8 hours of first paracetamol exposure
What is vital to take in the history?
- How much paracetamol and when?
- Anything else taken?
What may be given in very acute settings for paracetamol OD?
Activated charcoal
When can activated charcoal be given in paracetamol OD?
Within an hour
What is the main complication of paracetamol OD?
Acute liver failure and coagulopathy
