For the head injured adult pt, what is given to pre-medicate in RSI?
Liodcaine 1 mg/kg IV
Why is lidocaine given in the adult head injured pt for RSI?
IV lidocaine has been shown to preferentially constrict cerebral vasculature. This reduces ICP spikes, which is neuroprotective to the injured brain.
For the head injured adult pt, what is given for pre-existing bradycardia?
Atropine 0.01 mg/kg IV
For the head injured adult pt, what is the first line sedative agent?
Etomidate 0.3 mg/kg IV
For the head injured adult pt, what is the 2nd line sedative agent?
Ketamine 1.5-2 mg/kg IV
For the head injured pt, Ketamine is second line induction agent, as we do not want the ICP maintenance that Ketamine provides.
For the head injured adult pt, what are the 1st line induction agents ?
- Succinylcholine 1.5 mg/kg IV (100 mg minimum)
OR
- Vecuronium 0.1 mg/kg IV
For the head injured adult pt, what are the 2nd line induction agents ?
Succinylcholine 1.5 mg/kg IV (100 mg minimum) to induce paralysis followed by Vecuronium 0.05 mg/kg to maintain paralysis
Rapid Sequence Induction: “Induction” refers to?
