Serous
Clear, watery plasma
Purulent
Thick, yellow, green, tan or brown
Serosanguineous
Pale red, watery: mixture of clear and red fluid
Sanguineous
Bright Red: indicated active bleeding
Abrasion
Superficial wounds with little bleeding and is considered a partial thickness wounds. The wound often appears weepy because of plasma leakage from damaged capillaries.
Primary Intention
Wound that is closed
Secondary Intention
Wound edges are not approximated
Tertiary Intention
Wound is left open for several days, then wound edges are approximated
Laceration
Wound that is jagged, unintentional (non surgical wound), sometimes bleeds more profusely, depending on depth and location.
Dehiscence
Is partial or total separation of wound layers. The patient is at risk for poor healing.
Patients with poor nutritional status, infection, or obesity are at risk for dehiscence.
Evisceration
Protrusion of visceral organs of wound opening.
Hematoma
Is a localized collection of blood underneath the tissue. It appears as swelling, a change in color, sensation, or warmth, or a mass that often takes on a bluish discoloration.
Asepsis
Is the absence of pathogenic (disease producing) microorganism.
Abscess
A collection of pus in any part of the body. The area can be swollen and inflamed.
Adhesions
Are bands of scar-like tissue. Normally, internal tissue and organs have slippery surfaces so they can shift easily as the body moves. Cause tissues and organs to stick together.
