What is a review meeting?
It’s where the support of an individual is checked to see if it is still working/appropriate
Why are review meetings important?
Puts the individual at the centre of the meeting

Ensure care relationships are effective
What is the role of the facilitator?
Supports the person whose review it is by asking:
What are the person’s strengths and weaknesses?
What can be done to help this individual achieve their objectives?
What is important for this person in the future?
What is/isn’t working for this person at the minute?
How does the facilitator lead the meeting?
Invites contribution from everybody present
Helps the group explore their different perspectives
Assist the group in agreeing a common direction
Checks with the individual that they are happy with the way the review is going
How can the the individual be made to feel as comfortable as possible during the meeting?
They can invite whoever they wish
They can choose when the meeting should be
They can choose where the meeting should be
Meeting place should be accessible
