What is a good days/bad days tool?
An individual describes a typical good day so the professional can do more of that with them.
An individual describes a typical bad day so the carer can avoid doing those things
What is the routines tool?
Getting an individual to talk through their daily routine to provide an idea of what is and what isn’t working for them
What is the top tips tool?
A brief summary of an individual’s needs so a new carer can read in 2 minutes and know roughly what they should do to make sure the individual gets what is important to them
What is the relationship circle tool?
Help identify who an individual knows, how they know them and who is close to them. These networks can be used by the carer to identify gaps or help the individual see more of the people that mean more
What is the one-page profile tool?
Summary of what matters to the person. Strengths, what is important to them, how they like to be supported, what they don’t need help with. Especially useful if the individual finds it hard to express their needs

What is the communication chart tool?
Helps people who don’t use words to communicate. Tells carers what certain actions mean so that the individual doesn’t get frustrated when the care can’t understand them
What is the decision making chart tool?
Tells carers how much control the individual wants in terms of decision making. Which decisions they want to be a part of and how they need help to make those decisions
Why is building effective relationships good enhancing voice, choice and control?
Talking and listening to individuals builds trust so the individual is more likely to share their wants and needs. Especially if they feel they can rely on the carer to do what they say they will
What is the tool for clarifying roles and responsibilities?
Doughnut chart
What is the doughnut chart tool?
It clarifies who does what. It tells the carer what they MUST do, what they can use their judgement about and what ISNT their responsibility
