what is the difference between a re-assessment and a re-evaluation?
assessments we do all the time. at every intervention the patient’s status should be reassessed and documented.
re-eval refers to a formalized repetition of the initial evaluation
when is a re-evaluation done?
for formal reporting to payers, physician
after a major change in pt status
what are the two main forms of outcome measures? explain.
self-reported measures: surveys taken by patient ex. patient-specific functional scale, oswestry disability index
performance based measures: physical tests done by patient ex. berg, mmt, goni
when should you start to plan for discharge?
in the initial evaluation
what is reliability?
are the tests accurate
the tests contain internal consistency (all subsections correlate), reproducibility (score will be stable overtime), and agreement (repeating doesn’t result in change unless there has been a change)
what is validity?
test is measuring what we think we are measuring
the test contains the three Cs; content (measuring everything that should be measuring), construct (the measure matches the definition of what is being measures), criterion (instrument is compared to gold standard)
what is responsiveness?
minimal detectible change
what is MCD?
minimal detectible change: the amount of change that has to occur so that I know it is not by accident/error
what is MCID?
minimally clinically important difference
the amount of change that needs to occur to have meaningful clinical change
what is a global rating of change scale?
VAS 15, 11, or 7 pts
single question that asks patient to rate change with respect to a particular condition over a specific period of time

what is the pt ready for? what does the next step need to be? what do they want it to be?
what is the order of the continuum of care? will patient’s always follow this order?

they will progress through the order but may skip steps
