Birthing Center

Acute-Care Hospitals
Treatments of brief but severe episode of illness, for conditions that are the result of symptoms, disease, trauma, or recovering from surgery.

Primary care, specialty care, and medical home care in an outpatient setting
Retail health clinic
Primary care services provided in a a walk-in clinic located within a retail operation
Urgent Care
Treatment for non-life threatening illness and injuries, typically open seven days a week with hours extending into the evening for unscheduled care requiring immediate attention
Emergency Care
Medical-surgical care for injuries or illness that could be life threatening or disabling if not treated immediately

Preventative, diagnostic, and therapeutic services provided on a nonresidential basis

Surgical and diagnostic medical services provided in a freestanding facility
Specialty hospital
Medical care provided to individuals who are clinically complex with multiple acute and chronic conditions requiring an average length of stay greater than 25 days
Behavioral Health
Psychiatric and psychological care provided to address mental disorders, developmental disorders and substance-abuse disorders
Rehabilitative services
Therapeutic medical services (speech, physical, and occupational therapy) provided to patients who have been disabled by injuries or illnesses, with the goal of helping patients recover as much function as possible
End-stage renal disease treatment facility
Outpatient treatment facility that provides dialysis treatment, maintenance, and self-care training.
Health home care
Medical and/or personal care provided to individuals and families in their place of residence with the goal of promoting, maintaining, or restoring health or minimizing the effects of disabilities and illnesses, including terminal illnesses
Assisted Living
Facilities that provide assistance with activities of daily living
Intermediate care facility
Medical care that is above the level of custodial care but not at the level provided at an acute-care facility or skilled nursing facility
Skilled Nursing Facility
Medical, nursing, social, and rehabilitative services for persons that require 24-hour nursing care and supervision
Hospice
Palliative and supportive care of terminal conditions where life expectancy is six months or less
Long-term acute care
the medical care provided to individuals who are clinically complex and have multiple acute and chronic conditions requiring an average length of stay greater than 25 day in a long-term care hospitals verified as acute-care hospitals
Long-term care
the medical and/or personal care services provided to chronically ill, aged, disabled, or intellectually challenged individuals who reside in dedicated nursing facilities
Acute care
short-term medical and nursing care provided in an inpatient hospital setting to treat the acute phase of a patient’s injury or illness
