A

by other drugs like clarithromycin- less metabolism by P450 enzymes as competing for same site: also with age ie less GFR+ metabolism, and malnutrition (less plasma proteins)

food intake (better on empty stoamch), fluid (helps with absorption), age (newborns and elderly less metabolism/excretion), NON-COMPLIANCE, other diseases like GI disorders (less absoprtion), nutrition
