Treatment of cardiac rhythm disturbances Flashcards
3 types of rhthym disturbances?
too fast
too slow
disorganised
What can pacemakers be used for?
only for bradycardia
AV syncrony - AV block normally
VV syncrony
Defibiraltor
VT
CRT - cardiac resynchronisation treatment
cardiomyopathy and left bundle branch block
Common treatment for rhythm disturbances
-assessment, reassureance, drug therapy, withdrawl of drug, management of underlying condition, device, intervension
Lidocaine
sodium channel agents -
Flecainide
sodium channel agents
amiodarone
potassium channel blocekr
- can potentiate wafarin and get a very high INR
- need to figure out dose, and also if maybe put this on
Long QT syndrome = aquired
- the patient is taking something to prolong QT interval
- maybe need pacemaker
aV nodal re-entrant tachycardia
- can try and slow the traffic through the AV node via a beta blcoekr, calcium channel blocker
- could also given adenosine to block the av - need to be careful with dose
AV re-entrant tachycardia
- via an accesroy pathway
- if it goes throguh pirkinjisystem first then will be a narrow complex
- and if goes clockwise then will be broad
Atrial fibrillation
most people dont realsie they have this, until they have symptoms such as stroke
- need to reduce the risk of stroke by regulating cardiomyopahty
- test stroke risk by anticoagulation
- wafarin, aspirin, heaparin, dabigatran - increase risk of bleeding
- want to slow rate of atrial to ventricle propgation (AV node)
- beta blockers
- calcium channel blocker
- amiodarone
pacemaker -to burn out av node
AF ablation
-try to damage and stop the thing that is triggering this
Ventricular tachycardia
-pacemaker, icd, bb,
potassium channel blocker - amiodarone
- blue/red appearance of cheeks
- many side effects of this - pulmonary fibrosis , liver failure, renal failure
