Drugs for idiopathic Pulmonary Fibrosis
For mild and moderate idiopathic pulmonary fibrosis
Pirfenidone = Reduces disease progression and preserves lung function.
Nintedanib= slows the disease progression.
Pirfenidone
Drug for Idiopathic pulmonary fibrosis. Slows the progression.
Inhibits transforming growth factor beta (TGF beta) ( this is for scar formation)
Blocks fibroblast proliferation and collagen synthesis
decreases the extracellular matrix
Side effect:
1. Photosensitivity and rash
2. Elevated liver enzymes
Nintedanib
Drug for Idiopathic pulmonary fibrosis. Reduced the progression
Inhibits multiple receptor tyrosine kinases for example:
1. Platelet derived growth factor receptor
2. Fibroblast growth factor receptor
3. Vascular endothelial growth factor receptor
Blocks fibroblast activation and proliferation
Side effect:
1. Diarrhea, nausea and vomiting
2. Elevated Liver enzymes
3. increase risk of arterial thromboembolic events
4. Bleeding risk due to inhibition of VEGF
5. Fatigue and weight loss

- Bleomycin
- Busulfan
- Methotrexate
- Amiodarone +++++
- Nitrofurantoin
- Leflunomide
- Infliximab, etanercept, adalinumab
- gefitinib and erlotinib
